Can you tap and hold on the icon to move it around, or is it stuck in the middle of the screen and you can see it in applications as well?
Make sure that you have not enabled something in Settings > Accessibility or that you have enabled the Developer options that can be found in Settings > System > Developer options.
If you could also take a screenshot of how it looks, that would be great. To do that, hold the power button and tap on "Take screenshot" and upload it here.
